Chelsey Lesniak, MSN, APN, FNP-BC
Chelsey is a family nurse practitioner with full practice authority, she is thrilled to be the owner and Primary Care Nurse Practitioner at Thrive Healthcare. Chelsey loves providing passionate, patient-centered care on your terms.
Chelsey began her career as a paramedic and has an extensive background in emergency and intensive care medicine, flight medicine and medical aesthetics. She was an emergency room RN for over 15 years and a flight nurse with Flight for Life McHenry for 5 years. She graduated magna cum laude from Purdue University in 2018 with a Master’s of Science in advanced practice nursing. Chelsey completed an additional 4,000 hour clinical residency and obtained full practice authority in 2020. She also has over 250 post-graduate hours in endocrine and hormone management. She lives in Crystal Lake with her extremely supportive husband and four children. In her spare time she loves to pretend to be a farmer, is usually knee deep in a random project and loves to travel off the beaten path.
Jesse Freidel, MSN, APN, FNP-BC
Jesse has long had a passion for wellness and caring for those around him. He has had the opportunity to implement this while working in the emergency room and as a flight nurse. He received his BSN at Purdue University and his MSN at Chamberlain College of Nursing. He is a board certified family nurse practitioner and has most recently worked as a pediatric nurse practitioner. His love for flying means that he also continues to work as a flight nurse. He is married and the father of five busy kids — they have a variety of animals including dogs, cats, turkeys and chickens. He loves to live an active lifestyle and spend time with his family wherever they go.
Jami Kordecki, CCMA Office Manager
Jami is a certified medical assistant, the office manager and jack of all trades. She is the heart of the practice and all of our patients know and love her. Jami worked as a medical assistant for a large primary care practice until she started a family; after taking an 8 year break to stay home with her boys, she returned to the office setting and quickly became indispensable. Jami lives in Crystal Lake with her husband and two boys, in her spare time she loves to hang out with her husband and kids, spend time with her extended family or have a glass of wine and date night.
Kayla Brunetti, Medical Assistant
Kayla is new to the medical assistant role, she has a background as a medical scribe and has jumped into her new position at Thrive. She will be helping Jami as she learns the ropes of a busy primary care office. Kayla lives in Woodstock with her husband and two boys, in her spare time she loves to spend time with her family, read personal development books, and be on the soccer field cheering on her boys.
Sarah Papic, Paramedic
Sarah has a background in emergency medicine and also works as a firefighter/paramedic in McHenry County. Her passion for caring, shows through her warm personality and love for building strong relationships.
She enjoys working for an office that has a multifunctional approach to healthcare. From primary care options, to services that promote wellness, Thrive helps everyone to feel their best and Sarah enjoys being a part of that vision.
For fun she enjoys walks with her dog and playing violin.
Amanda Hurley, Social Media Manager
Amanda is a registered nurse and social media savant. She works full-time as a nurse and also runs social media and advertising for Thrive. Amanda has been a key part of the growth at Thrive Healthcare and loves to keep our personality at the forefront of our social media posts. You’ll never find her without her phone – usually taking pictures for her insta account. Amanda lives in Spring Grove with her husband and two year old daughter. In her spare time she enjoys organizing, working out, entertaining and cooking.
Linda Yin, CCMA
Linda is a certified medical assistant, she is currently in nursing school and helping out in the office as Thrive Healthcare continues to grow. She has an extensive background in family practice, immediate care and hospital medicine. Linda lives in Crystal Lake with her husband, four kids – ranging from 16-2 years old, and her crazy Goldendoodle. In her free-time she enjoys being outdoors with her kids, shopping/browsing the local farmer’s market, and crafting but her husband would probably say she enjoys annoying him instead during her free-time.
Jill Raybourne, RN, BSN
Jill is a Registered Nurse joining our team with 7 years of labor and delivery experience and 13 years of internal medicine experience at a large Crystal Lake practice. Jill graduated from MCC in 2016 and Western Governors University (BSN) in 2024. She lives in Algonquin with her husband and blended family. She has 5 kids that range from ages 3-23 years. She enjoys cooking, gardening, snowmobiling, and riding motorcycles with her husband. In her free time she enjoys attending as many country concerts as possible and spending time with family.

What are your hours?
We are closed on most D47 holidays, institute, and early release days.
What insurance do you take?
We also have reasonable cash pay rates for those without insurance.
Do you give vaccines in the office?
Vaccines are not administered at our office at this time.

How long is a typical appointment?
Because we truly want to provide enough time to hear all your concerns and thoughts, you can plan to be in our office for about an hour.
Is Chelsey like a regular doctor or is she more holistic?
Will I be pushed to take a medication?
All treatment plans are collaborative and are created to suit your lifestyle and beliefs. We will never pressure you into a treatment plan you’re not comfortable with.
